- Aetna (HMO Requires Referral)
- Blue Cross Blue Shield (No HMO, no blue select, health options, premier, highmark or
IDs that start with TGH or TGM) - Cigna (HMO Requires Referral)
- Medicare
- Tricare (Tricare prime requires a referral from your primary care physician)
- United Healthcare
Dr. Mueller takes all the same insurance with a few additional plans:
- Careplus, Humana GoldPlus, Devoted (in Polk County only)
- All of these plans require a referral from the primary care provider.
