• Blue Cross Blue Shield (No HMO, no blue select, health options, premier, highmark or
  IDs that start with TGH or TGM)
• Cigna (HMO Requires Referral)
• Medicare
• Tricare (Tricare prime requires a referral from your primary care physician)
• United Healthcare

Dr. Mueller takes all the same insurance with a few additional plans:

• Careplus, Humana GoldPlus, Devoted (in Polk County only)
• All of these plans require a referral from the primary care provider.
